How to Prepare for your Orthopedic Visit with Pediatric Orthopedic Specialists

What items should I bring to my visit?

Parents:

  • ID card (+ your child’s if they are over 16 years old), Insurance Cards

 Patients:

  • Any imaging studies that are applicable to the visit. This includes X- rays, CT and/or MRI scans.
  • Scoliosis brace, if applicable
  • Wear or bring shorts if your doctor is going to need to assess your child’s lower legs.

Are there any forms to complete in advance?

Yes. When you set up your initial appointment, we ask you for your phone number and email address.  A link will be sent to fill out your initial patient intake along with consent forms. If you are unable to complete the link, we have a copy in the office for you.

Parking at the clinic

Salt Lake Clinic

Parking is best on the East Side of the building.  We have 14 guest parking spaces along the front of the building. Our office is the first door on the right side as you walk in. You won’t miss our child-friendly waiting room.

Park City Clinic

We are on the first floor of the Park City Intermountain Hospital.

  • When you arrive, either park in the hospital’s front parking lot or use the entrance on the right side of the building (under The Orthopedic Partners sign). Our check-in desk is directly across from the PT clinic.

Can you perform X-rays in the clinic?

Yes! We have 2 targeted low dose radiation Xray machines capable of taking all imaging in clinic.  We can do x-rays in as little as 5 minutes.
